# Employer–Provider Partnership Agreement (Starting Point)

**Parties:** [Employer], and [Provider], a licensed child care program in [city].

## 1. Arrangement (choose one)

- **Reserved slots:** Provider reserves [N] spots ([ages]) for Employer's
  employees. Employer pays a reservation fee of $[X]/slot/month whether or
  not filled.
- **Priority access:** Provider offers Employer's employees first access to
  openings before public waitlist placement. Fee: $[X]/month.

## 2. Payment and fees

- Reservation/priority fees invoiced monthly; families pay standard tuition
  [minus employer subsidy of $[X]/month if applicable, paid directly to Provider].
- Annual tuition changes: Provider gives [60] days' notice.

## 3. Term and review

- Initial term: [12] months, renewing annually unless either party gives
  [90] days' notice.
- Quarterly check-in: utilization, upcoming needs, satisfaction.

## 4. Standards

- Provider maintains licensing in good standing and notifies Employer of
  any change in license status within [5] business days.
- Employer makes no operational or curricular demands; Provider runs the program.

## 5. Confidentiality & non-endorsement

Family enrollment details stay between Provider and families. Neither party
uses the other's name in marketing without written consent.

## 6. Termination

Either party may terminate for material breach with [30] days' written
notice and opportunity to cure. Enrolled families retain their spots for
the remainder of the school year at standard tuition.
